Untuk Badan Usaha :

• Fotocopy NPWP, SIUP, TDP
• Fotocopy Akte Pendirian Perusahaan
• Fotocopy SK Menteri Kehakiman
• Fotocopy Rekening Koran 3 bulan terakhir
